This study aimed to clarify the prognostic impact of corticosteroid administration on clinical outcomes in dual ICI combination therapy (i.e., immunotherapy [IO]-IO) and combinations of ICIs with tyrosine kinase inhibitors (TKIs) (i.e., IO-TKI) for RCC. METHODS: We retrospectively evaluated the clinical data of 194 patients who received the IO-IO or IO-TKI as first-line therapy. The patients were divided into two groups according to corticosteroid administration requirements within the initial three months of treatment. Between the two groups, overall survival (OS) and progression-free survival (PFS) were compared in each treatment group. RESULTS: Overall, 101 (52.0%) and 93 (48.0%) patients received the IO-IO and IO-TKI, respectively. Among them, 32 (31.7%) and 9 (9.7%) patients in the IO-IO and IO-TKI groups received corticosteroids within the initial three months, respectively. In the IO-IO group, OS was significantly longer in patients receiving early corticosteroids than those without (median: 77.6 vs. 44.2 months, p=0.0230), and PFS was not significantly different between the two groups (median: 10.2 vs. 6.1 months, p=0.1272) (Figure 1). Subsequent multivariable analysis showed that corticosteroid administration was an independent predictor of a longer OS (hazard ratio: 0.381, p=0.0329). In the IO-TKI group, both OS and PFS were not significantly different regarding corticosteroid administration (p=0.495, p=0.169) (Figure 2). CONCLUSIONS: The corticosteroid administration did not decline the treatment effectiveness of ICI-combination therapies for advanced RCC despite the regimens.
