METHODS: A total of 20 preoperative and 18 postoperative patients with congenital megaloureter underwent cine MRU between July 2019 and February 2023. Preoperative patients were divided into the endoscopic group and the reimplantation group based on the treatment, and patients with bilateral involvement were included as two cases. Cine MRU captures continuous ureteral images over a short time based on T2-weighted images (Figure 1). From the ureterovesical orifice to the ureteropelvic junction, the ureteral contraction ratio, defined as amplitude/maximum diameter, was measured every 10% of the total length of the ureter, and those at the same level were grouped. RESULTS: There were 23 cases and 19 cases in preoperative and postoperative groups, respectively. The endoscopic group and the reimplantation group included 16 cases and 4 cases, excluding the three patients who did not undergo surgery. In the preoperative group, contraction ratios at 10% level and 20% level were significantly lower than those at 40% to 100% levels. In the postoperative group, there was no significant difference in the contraction ratio among the ten levels. Contraction ratios at 10% level and 20% level in the preoperative group were significantly lower than those at the same level in the postoperative group (19.55%±8.67% vs 31.51%±13.96%, p=0.002; 24.90%±9.78% vs 32.69%±11.87%, p=0.02; Figure 2). Thus, 20% of the distal ureter was considered to be functionally impaired. The area under the ROC curve in predicting whether endoscopy or reimplantation is more suitable was 0.875 (p<0.001) based on the preoperative ureteral contraction ratio at 20% level. The cut-off value was 13.66% and the Youden Index was 0.6875. CONCLUSIONS: Cine MRU can effectively assess the degree of functional impairment of congenital megaloureter and guide the following treatment choice.
