Background: Gynecomastia is defined as an abnormalgrowth of male breast tissue that is considered the most frequentmale cosmetic problem, accounting for roughly 60% ofthe total male breast problems and 85% of male breast masses.Several traditional treatments, including liposuction, subcutaneousmastectomy, and a combination of both, have beendocumented. This approach inevitably results in a scar on theanterior chest wall; liposuction alone can result in minimalscarring but has a minimal effect on the tough glandular andfibroconnective tissues. Objective: Evaluation of the aesthetic outcome of glandularliposculpture using V-dissector liposuction cannula forgrades I and II gynecomastia combined with the sealing effectof autologous platelet gel. Methods: This was an uncontrolled clinical study, including25 patients [50 breasts] ageing between 18 and 45 yearswith idiopathic benign gynecomastia who requested surgicalintervention for their gynecomastia. liposuction and liposculpturingwith V-dissector liposuction cannula without glandulartissue excision, followed by platelet gel spraying on the cavity. The procedure was completed, and the overall complicationrate was recorded, mainly hematoma and seroma. Conclusion: Glandular liposculpture using V-dissector liposuctioncannula for grades I and II gynecomastia combinedwith sealing autologous platelet gel was effective and safe, andboth patients and observers were adequately satisfied with anacceptable outcome rating.
