We present a 57-year-old male patient with keratitis, he was initially started on topical 0.5% moxifloxacin eye drops, but his symptoms did not resolve and infiltrate continued to increase. Corneal scrapings and microbiological reports suggested the diagnosis of the fungus, Schizophyllum commune , the patient was then switched to topical 5% natamycin eye drops hourly for 1 week followed by eight times per day for 2 weeks and oral tablet fluconazole 200 mg twice daily, following which the infiltrate was reduced, hypopyon resolved and visual acuity improved after 2 weeks of treatment.
