Abstract Rectus sheath hematoma (RSH) is a rarely encountered condition, the development of which can be influenced by various factors. We report a case of an unusual presentation of a massive spontaneous rectus sheath hematoma in a 63-year-old patient with an underlying bleeding tendency linked to liver cirrhosis and hypersplenism. The hematoma is complicated by infection, as indicated by the patient’s clinical presentation and the air-fluid level observed on CT scan. The patient's deteriorating condition prompted the surgical team to opt for percutaneous drainage as a therapeutic intervention, aiming to alleviate the infection within the hematoma.
