Free-wall rupture (FWR) is a catastrophic complication of acute myocardial infarction, generally treated with surgery.Reports of survivors of post-infarction FWR treated with conservative management are rare in the literature. We report a case of left ventricular FWR documented in a patient presenting cardiac tamponade (hypotension, muffled heart sounds, pulsus paradoxus, and jugular venous distension) who survived with medical treatment alone and showed spontaneous closure upon follow-up.Rupture size, symptoms, hemodynamic stability, and surgical risk determine the conservative treatment choice for left ventricular FWR.Close echocardiographic follow-up is paramount in these patients.
