Osimertinib has drastically transformed the landscape of targeted molecular therapy in epidermal growth factor receptor (EGFR)-mutated non-small cell lung cancer . Unfortunately, nearly all patients experience progression at some time regardless of which EGFR tyrosine kinase inhibitors (TKI) is used. The true challenge and question at hand is how to treat these patients following EGFR TKI failure. At the time of resistance, non-targeted approaches, such as chemotherapy, have proven clinical benefit. Targeted approaches to overcome EGFR resistance are being actively studied in ongoing clinical trials. The recent striking results of the addition of chemotherapy to osimertinib from the FLAURA 2 trial anticipate being practice changing for clinicians.
