Objective: To analyze the temporal and spatial distribution and associated factors of congenital syphilis in the municipalities of the state of Ceará from 2010 to 2019. Methods: Ecological study. Data from the Notifiable Diseases Information System was used. The Joinpoint method was used for the analysis of temporal trends. Spatial relationships were assessed using the calculation of the global Moran index, local Moran, and spatial scan statistics. Results: A stationary trend in the incidence of congenital syphilis was observed. Significant increases were noted in prenatal care, prenatal diagnosis, and partner treatment. Fortaleza showed the highest relative risk, with 3.49 times greater risk of congenital syphilis compared to other municipalities. Per capita income and life expectancy were positively related to the incidence of congenital syphilis. Conclusion: An increase in the incidence of congenital syphilis was observed in metropolitan areas, with a positive correlation with income and life expectancy.
