This article reviews pharmacological interventions for treatment-refractory depression in youth, highlighting diagnostic challenges and innovative approaches.
Depression that is difficult to treat in youth is a significant clinical challenge with profound impact on development, morbidity, and mortality. This article examines the current evidence base for pharmacological interventions in pediatric treatment-refractory depression, summarizing established approaches and emerging treatments currently being studied. Despite an alarming rise in major depressive episodes among adolescents, research addressing treatment resistance in this population remains limited, in part due to diagnostic challenges and heterogeneity. The article reviews diagnostic considerations, proposes a treatment algorithm, addresses safety concerns, and considers strategies to manage pediatric major depressive disorder and co–occurring conditions over time. It concludes by exploring innovative future directions, including novel approaches in pharmacological targeting, neuromodulation techniques, platform trials, and personalized medicine. Expanded research efforts are urgently needed to develop an evidence base for sequential interventions for the substantial proportion of youth who live with persistent depression and do not respond to first-line treatments for this serious and chronic condition.
