Case report demonstrates successful angioplasty and stenting managing hypertension in Takayasu arteritis patients.
Takayasu arteritis (TA) is a chronic, granulomatous large-vessel vasculitis predominantly involving the aorta and its primary branches. We discuss a case of a young female who presented with progressive dyspnea, orthopnea, and resistant systemic hypertension. 2D ECHO revealed features of DCMP with a reduced LVEF of 20–25%. CT imaging and angiography demonstrated involvement of abdominal aorta, bilateral renal arteries, and the superior mesenteric artery. Renal angiography revealed severe bilateral renal artery stenosis. A final diagnosis of Takayasu arteritis, complicated by DCMP and bilateral renal artery stenosis , was established. The patient underwent percutaneous transluminal angioplasty with drug-eluting balloon (DEB) in the left renal artery and bifurcation stenting with kissing balloon inflation in the right renal artery. This case highlights timely intervention in Takayasu arteritis leading to reversal of LV remodeling and control of resistant hypertension.
