Abstract This study compared the recommendations for the diagnosis and management of mitral stenosis (MS), a leading cause of valvular heart disease, as outlined in the 2021 European Society of Cardiology/European Association for Cardio-Thoracic Surgery guidelines and the 2020 American College of Cardiology/American Heart Association guidelines. A systematic review of the guidelines was conducted to inform clinical judgment. The preferred approach for diagnosing and grading MS involves a multimodal strategy that encompasses transthoracic and transesophageal echocardiography, assessment of evolving symptoms, hemodynamic evaluation via cardiac catheterization, and exercise testing. Both guidelines converge on the use of anticoagulation therapy, which has demonstrated efficacy in reducing thromboembolic events associated with rheumatic MS. However, a nuanced difference exists between the guidelines in terms of the criteria for assessing MS severity.
