Infective endocarditis (IE) is a life-threatening condition, particularly in individuals with congenital valvular anomalies.We present a case of severe IE in a patient with a bicuspid aortic valve (BAV), where echocardiography identified an aortic vegetation and an aneurysm of the anterior mitral leaflet.A 29year-old male with BAV and aortic regurgitation presented with systemic symptoms and hematuria.He was diagnosed with acute kidney injury, with a creatinine level of 7.18 mg/dL and a glomerular filtration rate (GFR) of 9 mL/min/1.73m,requiring hemodialysis.Echocardiography revealed severe IE with an anterior mitral leaflet aneurysm, significant mitral and aortic regurgitation, and aortic valve vegetation.Transesophageal echocardiography (TEE) confirmed an aneurysm and aortic vegetation.Given the extensive valvular destruction, urgent surgical intervention was planned for aortic and mitral valve replacement.This case highlights the severe and rare complications of IE in congenital valvular disease, emphasizing the importance of early diagnosis, multimodality imaging, and a multidisciplinary treatment approach.IE can present with glomerulonephritis, and in patients with congenital valvular disease, IE should always be considered.
