INTRODUCTION: To compare the effects of sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) on obesity-associated hypertension and weight loss in Chinese patients. METHODS: We systematically searched the Cochrane, PubMed, Web of Science, Scopus and Embase databases for studies comparing SG and RYGB, focusing on obesity treatment and blood pressure improvement, up to October 2025. Two independent authors extracted data, assessed quality, and statistically analyzed the included studies, The risk if bias was assessed using Cochrane 's tool for randomized trials and the Newcastle-Ottawa Scale for cohort studies. RESULTS: Fourteen studies (two randomized controlled trials and twelve cohort studies) encompassing 3234 patients (1725 SG, 1509 RYGB) were included. There was no significant difference in overall excess weight loss (%EWL) and total weight loss(%TWL) between SG and RYGB (MD=-1.55, 95%CI -6.64,3.53, P > 0.05) and (MD = 2.09, 95%CI -0.92,5.09, P > 0.05).RYGB showed superior long-term (> 1 year) %EWL compared to SG (MD=-8.32, 95%CI -12.14, -4.49, P 0.05), regardless of age or follow-up duration. There were no significant differences in systolic or diastolic blood pressure improvements between the two procedures. CONCLUSION: Both procedures effectively improve hypertension remission in patients with obesity in China, with no significant difference between them. Regarding weight loss, while short-term outcomes are similar, RYGB achieves superior long-term %EWL, indicating its greater potential for sustained weight maintenance.
