Purpose: Breast-conserving surgery (BCS) is a common procedure for early stage breast cancer. However, postoperative volume deficits and contour deformities remain a challenge. Acellular dermal matrix (ADM) is used for cosmetic enhancement in partial breast reconstruction; however, studies on pulverized ADM are limited. This study evaluated the early surgical outcomes of pulverized ADM in patients who underwent BCS for early breast cancer. Methods: We retrospectively evaluated 30 patients who underwent BCS with pulverized ADM insertion at a single institution. Data on patient demographics, tumor characteristics, surgical details, and postoperative complications were collected. Results: All 30 patients underwent successful BCS with pulverized ADM insertion without major intraoperative complications. The mean weight of the resected breast tissue was 24.5 g, and the average hospital stay was 5.1 days. Surgical site infection occurred in 3.3% (1/30) of the patients, and seroma was observed in 3.3% (1/30). No other complications were reported, and no reoperation was required. Conclusion: Pulverized ADM appears to be a feasible and safe option for immediate volume replacement after BCS, with favorable early surgical outcomes. However, further large-scale studies with long-term follow-up and objective aesthetic assessments are required to confirm its durability and clinical benefits.
