Objective. To evaluate the effectiveness of Miladean (memantine+melatonin) in patients with mild cognitive impairment MCI of various origins in real clinical practice, including its influence on cognitive domains and sleep quality, as well as treatment satisfaction. Material and methods. A prospective non-interventional program was conducted from November 2023 to July 2024 in 36 regions of the Russian Federation. The study included 498 outpatients over 18 years old with MCI lasting more than 2 years who received Miladean at the dosage recommended in the prescribing information. Assessments were performed at baseline and after 8 weeks using a structured questionnaire on cognitive domains and sleep, a visual analogue scale (VAS), and the Likert scale. Results. The total cognitive impairment score decreased from 5.96±1.52 to 2.04±1.52 points (–66%; p<0.01). A significant reduction was observed in the frequency of memory, attention, executive function, information processing speed, speech, visuospatial, and motor skill impairments. The mean sleep disturbance score decreased by 3.16 points (p<0.001), and sleep quality improvement according to VAS reached 42%. Full or near-complete treatment satisfaction was reported by 94% of physicians and 97% of patients. Conclusion. The program demonstrated the clinical effectiveness and good tolerability of Miladean in routine practice.
