Chilaiditi syndrome is a rare condition characterized by the interposition of colonic loops between the liver and diaphragm, which may mimic surgical emergencies and cause a diagnostic pitfall in the emergency department. We report a 53-year-old female diagnosed by computed tomography and successfully managed conservatively. Awareness of this syndrome is essential for emergency physicians to avoid unnecessary surgical interventions.
