The most applied diagnostic modality in LM patients in the Netherlands is punch biopsy, and most LM patients are treated with wide local excision in line with the Dutch melanoma guidelines. In this nationwide database, 14% of the histologically proven LM cases were upstaged to LMM after surgery. This should be taken into account when choosing treatment modalities, especially when considering non-surgical treatment modalities. The significant number of recurrences among non-surgical therapies demonstrates the complexity of treating LM and the importance of balance between histological clearance and sometimes mutilating surgery in the head and neck region.
