Abstract Background Patients with ulcerative colitis (UC) have more likely to experience depression and anxiety, especially during active disease. However, longitudinal data on the link between UC disease activity and depressive symptoms are limited. A prospective patient registry “YOURS” consisting of over 2,000 UC patients was established to explore how lifestyle and psychosocial factors affect clinical and mental health outcomes. This study aimed to examine the association between UC disease activity and the onset of depressive symptoms over 3 years. We explored modifiable risk factors for contributing to the onset of depressive symptoms. Methods Patients aged ≥16 years without depressive symptoms at baseline from YOURS registry were included. Baseline assessments covered demographics, disease activity (partial Mayo score), clinical characteristics, and lifestyle factors, and validated patient-reported outcomes, including the Hospital Anxiety and Depression Scale-Survey (HADS-D), Modified Medical Outcomes Study Social Support Survey (mMOS-SS), and Japanese version of the Perceived Stress Scale (JPSS).
