Comprehensive echocardiographic assessment of aortic regurgitation requires integration of multiview imaging and Doppler data with left ventricular remodeling to accurately guide clinical decisions.
Why the study?
Accurate transthoracic echocardiographic evaluation of aortic regurgitation requires optimized imaging techniques and Doppler assessment to guide clinical decision-making.
Provides a practical guide for optimizing transthoracic echocardiographic evaluation of aortic regurgitation to avoid common pitfalls and accurately guide clinical decisions.
Supports comprehensive AR assessment in complex cases; leaves open prospective validation against clinical outcomes.
OBJECTIVE: We provide a practical, step-by-step guide for sonographers, trainees, and allied professionals to help with transthoracic echocardiographic evaluation of patients with aortic regurgitation (AR), emphasizing optimal imaging techniques and Doppler assessment. KEY STEPS: Multiple views are required in order to fully evaluate the degree of AR, including imaging in the parasternal long-axis, parasternal short-axis, and apical views, and in the suprasternal notch to evaluate the descending aortic flow for diastolic flow reversal. POTENTIAL PITFALLS: AR jet misalignment, eccentric regurgitation, poor acoustic windows, arrhythmias, and calcification can lead to underestimation of AR severity. TAKE-HOME MESSAGE: Comprehensive AR assessment requires integration of multiview imaging and Doppler data with left ventricular remodeling and function to accurately guide clinical decision-making.
