Abstract Periodontally accelerated osteogenic orthodontics (PAOO) technique has several advantages which include shorter treatment time, augmentation of bone, and improved structural integrity of periodontal tissue which makes it a boon for both adolescents and adult orthodontics. In cases with compromised periodontal health, this technique influences the entire periodontium by means of alveolar bone-grafting augmentation that ultimately increases the size of the bony alveolus. This case report aimed to represent the application of PAOO technique with piezoelectric decortication and bone grafting in patient undergoing fixed orthodontic mechanotherapy for the correction of malocclusion. The patient showed uneventful healing and no incidence of other possible complications postsurgery. The incorporation of decortication facilitated accelerated tooth movement and a notably reduced treatment period. In addition, piezosurgery proved to be highly effective in the PAOO approach.
