These findings indicate that TN is associated with functional impairment of the glymphatic system in the absence of overt perivascular structural abnormalities. This dissociation between functional and structural glymphatic measures challenges conventional neuroimaging paradigms in TN and highlights a previously underrecognized central mechanism.
