Abstract Background Molecular profiling and subsequent discussions in the Molecular Tumorboard (MTB) within the network of Centers for Personalized Medicine (CPM) have expanded therapeutic options for neuro-oncology patients. Here, we present an update of our Tübingen MTB cohort in adult patients with advanced tumors in the nervous system. Methods The Tübingen MTB cohort is documented prospectively in a non-interventional observational study MTB@CPM (NCT03503149) focusing on precision medicine for patients with advanced tumors in routine clinical care. Following patient enrolment, molecular profiling was conducted, and targeted therapy recommendations were assigned after interdisciplinary MTB discussion. This was followed by approval of the MTB therapy by the neuro-oncology tumor board and its subsequent implementation. Results This is a follow-up and update of adult neuro-oncology patients in MTB@CPM (2016 through data cutoff 30 June 2024). From a total of 1088 patients, 854 patients were presented in the MTB. Molecular-instructed therapy was recommended for 773/854 (90.5%) patients and prioritized by evidence levels. Therapies were initiated in 152/773 (19.7%) patients, and a PFS ratio was available for 131/152 (86.2%) of treated patients with PFS2/PFS1 ratio 1.3 in 34.4% (45/131). Conclusions This follow-up demonstrates the feasibility of precision medicine for adult neuro-oncology patients within the well-structured and quality-assured MTB/CPM workflow and its implementation as an integral part of the clinical routine leading to a clinical benefit in about one third of all patients.
