Abstract Mandibular premolars are notorious for their unpredictable anatomical variations. While the dental literature frequently describes the presence of additional canals within mandibular premolars, reports of these teeth having extraroots are considerably rare. This paper aims to detail the successful endodontic treatment of an unusual case involving a two-rooted mandibular premolar. Various number of studies carried by different authors and were divided into anatomical studies reporting number of roots, number of canals, and apical morphology. Variations because of sex and ethnic background have also been reported. It will provide a comprehensive account, including diagnostic images, insights from during the procedure, and posttreatment radiographic evaluations. Furthermore, the article will present the significant data regarding the overall occurrence of extra roots in these teeth. The presence of these additional roots was confirmed using the standard radiographic assessment techniques.
