Abstract Visual loss in haemorrhagic occipital infarction following acute ischemic stroke (AIS) can be sudden and devastating. We present the successful management of a 50-year-old man, who had suffered an AIS 2 years ago following the first dose of the ChAdOx1 nCoV-19 vaccine (COVISHIED) and developed bilateral occipital infarction, oculomotor palsy and subsequently cataract. He underwent bilateral uneventful small incision cataract surgery (SICS) and posterior chamber intraocular lens implantation and had a favourable visual outcome. It is certainly worthwhile performing cataract surgeries on these patients and SICS is a useful option in a low resource setting.
