Abstract We report the case of a young male patient, who developed dense multilayered vitreoretinal haemorrhage, including sub-internal limiting membrane (ILM) and subhyaloid haemorrhage, following a road traffic accident with associated subarachnoid haemorrhage. Prompt pars plana vitrectomy (PPV) with ILM peeling and evacuation of sub-ILM haemorrhage resulted in excellent anatomical and visual recovery. The case highlights the importance of early surgical intervention in post-traumatic haemorrhagic retinopathy to prevent phototoxic damage and irreversible visual loss.
