This review provides a guide to the pathology of the most recent classification of inflammatory bowel disease-associated neoplastic precursor lesions. Its objective is to orient clinicians to the morphological and terminological framework underlying this system. It outlines the historical basis and rationale for the updated classification; describes the morphologic, endoscopic, and molecular features of newly recognized neoplastic precursor subtypes; and summarizes the emerging, though still incomplete, evidence linking these features to clinical outcomes. By placing current findings in context and highlighting persisting knowledge gaps, we provide clinicians with a conceptual foundation for interpreting the new classification and understanding its potential implications for surveillance and therapeutic decision-making.
