Online health communities offer a supportive environment for individuals with shared health concerns, providing emotional support and a platform for the exchange of information. However, they also present challenges, such as the dissemination of inaccurate information and the potential for negative social interactions, which can affect decision making and have an impact on health outcomes. This article explores the dualistic nature of these communities, examining the factors driving engagement and the resulting consequences, emphasising the need to leverage their potential to improve public health outcomes. Within this, the authors argue that over-engagement with these communities can paradoxically transform their potential benefits into disadvantages. Directions for future research to examine this paradox and ways to ensure that the growing use of online health communities can have a positive impact on public health are discussed.
