Abstract: Dr. Roman Castroviejo began performing square penetrating keratoplasty in the early 1930s. We present a case of one of his grafts that underwent successful cataract surgery 60 years after the keratoplasty was performed. The graft remains clear and stable at 12-month follow-up. This is a remarkable result for a keratoplasty to remain free from failure 60 years after transplantation.
