Modern dentistry has expanded remarkably through advanced imaging, digital workflows, artificial intelligence (AI), and machine learning (ML)-driven diagnostics. While diagnostic precision and treatment outcomes have improved significantly, structured and retrievable dental record systems—particularly in busy rural private practices—have not evolved at the same pace. This gap presents challenges from a forensic identification perspective. Rather than attributing blame, this article proposes a simplified, minimal, and practical forensic-oriented documentation framework. By embedding structured habits at the undergraduate level, integrating one-page minimal record templates into routine practice, and encouraging adoption through professional incentives, rural clinics can develop reliable micro-databases for forensic identification. Sustainable change in record keeping requires simplicity, early training, and supportive motivation.
