Case report reveals postoperative paralysis in a young girl due to functional neurological disorder, suggesting need for early intervention.
Case: A 13-year-old girl with acetabular dysplasia developed global paralysis of the left lower limb after undergoing a left periacetabular osteotomy. Electromyography revealed no nerve injury. However, symptoms persisted for 3 months. Psychiatric evaluation confirmed a functional neurological disorder diagnosis. Multidisciplinary treatment led to complete recovery. Conclusion: Functional neurological disorder may present as postoperative paralysis after periacetabular osteotomy, mimicking nerve injury. Early recognition based on positive clinical signs and prompt multidisciplinary management is essential to prevent unnecessary interventions and achieve full functional recovery. Awareness of this infrequent entity is indispensable when evaluating postoperative motor deficits.
