Abstract

虐待による頭部外傷 (abusive head trauma : AHT) は虐待死の死因として最も多く, 深刻な社会問題となっている. 本稿では, 本邦におけるこども虐待統計の推移, AHTの歴史的背景, AHT予測ツールとして海外で開発されたPediBIRNとPredAHTの役割と課題, AHTと刑事裁判について概説し, 最後に髄鞘化の未熟な乳児期特有の脳損傷として皮質下白質裂傷とTBIRD (infantile traumatic brain injury with a biphasic clinical course and late reduced diffusion) を紹介する.
