Objectives: This study aimed to compare diode laser-assisted and conventional surgical circumferential supracrestal fiberotomy in preventing rotational relapse following orthodontic derotation. Methods: Seventy-six orthodontic patients were randomized into control, conventional fiberotomy, and diode laser fiberotomy groups. Rotational relapse and irregularity index were measured using 3D scanned models. Results: Both fiberotomy groups demonstrated significantly reduced relapse compared to controls (p < 0.001). No statistically significant difference was observed between diode laser and conventional methods. Conclusions: Diode laser-assisted fiberotomy is equally effective as conventional surgical techniques in reducing orthodontic relapse.
