Abstract

This guide examines why healthcare AI systems that perform well in pilots often fail to scale in real-world settings. Moving beyond model accuracy and technical performance, it explores the systemic, operational, and human factors that determine whether AI can be sustained within everyday clinical and organisational workflows. Drawing on recurring patterns observed across deployments, the guide outlines ten core reasons why AI initiatives stall between pilot success and real-world integration. These include infrastructure gaps, data fragmentation, workflow misalignment, unclear ownership, delayed governance, compliance design challenges, and economic constraints. Each is presented not as an isolated issue, but as part of an interconnected system where technology, people, processes, and incentives shape outcomes collectively.
