Endurance exercise in CVD is linked to elevated small and large cTnT proteoforms versus healthy individuals.
Why the study?
Cardiac troponin T elevations are common after strenuous exercise and may present a sign of cardiac vulnerability, prompting exploration of associations between cTnT proteoforms and cardiovascular disease.
Does the profile of cTnT proteoforms released after endurance exercise differ between individuals with and without cardiovascular disease?
Observational (n=6)
Does the profile of cTnT proteoforms released after endurance exercise differ between individuals with and without cardiovascular disease?
Post-exercise release of large cTnT proteoforms (29 kDa) may distinguish individuals with underlying cardiovascular disease from healthy athletes who only release small proteoforms (<20 kDa).
Small cTnT proteoforms may not signal vulnerability post-exercise; hypothesis-generating for proteoform-guided interpretation of troponin elevations in athletes.
Cardiac troponin T (cTnT) elevations are common after strenuous exercise and may present a sign of cardiac vulnerability. From our derivation cohort of long-distance walkers (n=804), we re-analyzed blood samples from six individuals exhibiting very high post-exercise cTnT concentrations using Gel Filtration Chromatography to explore associations between cTnT proteoforms and cardiovascular disease (CVD).Healthy individuals demonstrated elevated concentrations in small cTnT proteoforms (<20 kDa), whereas individuals with CVD demonstrated elevated concentrations of both small and large cTnT proteoforms (29 kDa).Our findings suggest release of distinct cTnT proteoforms between individuals with versus without CVD, promoting further investigation in recreational athletes.

Luiken et al. (2026) conducted an observational in Post-exercise cTnT elevation (n=6). Endurance exercise (long-distance walking) vs. Healthy individuals vs individuals with cardiovascular disease was evaluated on cTnT proteoforms. Endurance exercise in individuals with cardiovascular disease resulted in elevated concentrations of both small (<20 kDa) and large (29 kDa) cTnT proteoforms, unlike healthy individuals.
