学童期に見つかった食物アレルギーの食事制限を契機に発症したビタミンd欠乏性くる病の1例

Abstract

【症例】12歳男児.受診半年前から両膝痛,両足関節痛が出現し,手すり無しでは階段昇降が困難となり当院受診.既往にアトピー性皮膚炎,上肢骨折(計4回),食物アレルギー(卵・牛乳・小麦・魚)があり,普段から米と肉類を主に摂取していた.単純X線像にて足関節の骨端線の開大,骨端部の骨梁の粗を認め,採血にて低Ca血症,ALP上昇,25-OHビタミンDの低下を認めた.食事制限によるビタミンD欠乏性くる病の診断にて活性型ビタミンD,乳酸Ca内服を開始した.内服2ヵ月後で疼痛は軽減し,3ヵ月後の単純X線では骨端線の縮小を認め,10ヵ月後にはCa,ALPともに正常化,2年半後には骨端線は閉鎖し,痛み無く運動も可能である.【考察】近年,食物アレルギーによる食事制限が原因で幼少期にくる病を発症した報告が散見されるが,学童期の報告はまれである.食物アレルギーの既往のある学童期の関節痛を診た場合,くる病も鑑別に挙げる必要がある.
