Heart failure with reduced ejection fraction (HFrEF) is common in end-stage renal disease (ESRD) and carries high morbidity and mortality. Sodium-glucose cotransporter-2 inhibitors (SGLT2i) are foundational therapy for HFrEF, but pivotal trials largely excluded ESRD, leaving uncertainty about real-world use in this population.
