The coexistence of an aortoiliac aneurysm (AIA) and a congenital pelvic kidney is a rare pathology posing a significant challenge in surgical treatment. This paper presents the case of an 84-year-old male patient who was admitted to the Department of Vascular Surgery due to a progressive AIA. Based on clinical presentation and imaging studies, the patient was qualified for customized stent graft implantation. The patient underwent the procedure without complications and was discharged home in good general condition. This case emphasizes a customized endovascular approach as a valuable treatment alternative for patients with complex anatomical aberrancy, such as AIA and congenital pelvic kidney.
