Introduction Real-world ocular adverse events and collaborative management are described in a representative patient with platinum-resistant ovarian cancer treated with mirvetuximab soravtansine-gynx, a folate receptor alpha-targeting antibody-drug conjugate that showed significant survival and clinical benefits over chemotherapy in the MIRASOL trial. Case Report A representative case was selected from 26 mirvetuximab soravtansine-gynx–treated patients with platinum-resistant ovarian cancer referred to the Beth Israel Deaconess Medical Center Ophthalmology Division. The patient was evaluated before initiation and after every other mirvetuximab soravtansine-gynx infusion, unless a more frequent follow-up was indicated and the patient received prophylactic topical corticosteroid and daily lubricating eye drops. The patient experienced epithelial keratopathy with grade 3 reduction in best-corrected distance visual acuity, necessitating a pause in treatment. Keratopathy improved thereafter, but recurred after reintroduction at a lower dose, leading to further dose reduction before findings eventually cleared and best-corrected distance visual acuity improved. Conclusion This case demonstrates the clinical course of ocular adverse events in a patient receiving mirvetuximab soravtansine-gynx with dose delay or adjustment.
