Implication

May flag subclinical LV impairment in asymptomatic beta-thalassemia; hypothesis-generating and requires prospective validation before clinical use.

Structured PICO

Do radionuclide angiography and heart rate variability analysis detect early cardiac dysfunction in asymptomatic patients with beta-thalassemia?

P
Population
20 patients (7 females; median 25 yr [first quartile 22, third quartile 28]) with beta-thalassemia with no symptoms or echocardiographic evidence of dysfunction at routine monitoring.
I
Intervention
Radionuclide angiography (RNA) at rest and during low-dose dobutamine infusion (5-10 gamma/kg/min) and 24-hour Holter monitoring for heart rate variability (HRV) assessment.
O
Outcome
Early myocardial damage assessed by right and left ventricular ejection fractions (EF) and heart rate variability (HRV).surrogate

Right ventricular dysfunction on radionuclide angiography and abnormal heart rate variability may serve as early markers of cardiac disease in asymptomatic thalassemic patients.
