Case report of corneal decompensation managed with DSAEK while retaining AC-pIOL, indicating effective management strategies.
To present a case of corneal decompensation several years post anterior chamber phakic intraocular lenses (AC‐pIOLs) surgery managed by DSAEK (Descemet stripping automated endothelial keratoplasty) with the AC‐pIOL left in situ . A 44-year-old female presented with decreased vision many years after initial AC‐pIOL surgery. On examination, she was diagnosed with corneal decompensation which was managed by DSAEK. Patients who have undergone AC-pIOL implantation must be monitored for endothelial cell loss. Leaving the AC-pIOL intact and performing just a corneal transplantation surgery can be considered as an option in cases of AC-pIOL-associated corneal decompensation, thus preserving the patient’s accommodation as well.
