Key result
Hemodialysis links to a ~20% lower prevalence of cardiac autonomic neuropathy across CKD stages.
Why the study?
Cardiac autonomic neuropathy is a significant complication in CKD leading to increased morbidity and mortality, making early detection essential, especially in patients undergoing hemodialysis.
Does Bellavere's Score accurately diagnose cardiac autonomic neuropathy in patients with chronic kidney disease?

Does Bellavere's Score accurately diagnose cardiac autonomic neuropathy in patients with chronic kidney disease?
Bellavere's Score provides a reliable, non-invasive method for diagnosing cardiac autonomic neuropathy across all stages of chronic kidney disease, and hemodialysis significantly improves autonomic function in these patients.
Answers are grounded in this paper and cite their sources.
Bellavere's Score may support non-invasive CAN diagnosis in CKD; leaves open hemodialysis effects on prevalence pending prospective data.
Background: Cardiac autonomic neuropathy (CAN) is a significant complication in chronic kidney disease (CKD), leading to increased morbidity and mortality. Early detection is essential for managing CKD patients effectively, especially those on hemodialysis. This study evaluated the prevalence CAN in CKD and diagnostic accuracy of Bellavere's Score in predicting CAN in CKD patients, including those undergoing hemodialysis. Methods: This prospective observational study included 200 CKD patients. Cardiac autonomic neuropathy was assessed using Bellavere's Score, calculated through a series of autonomic function tests including heart rate variability and blood pressure responses. Bellavere's Score was measured pre-and post-dialysis in hemodialysis patients. The diagnostic performance of the score was evaluated using receiver operating characteristic (ROC) curves to determine sensitivity, specificity, and likelihood ratios. Conclusion: Bellavere's Score provides a reliable and non-invasive approach for diagnosing CAN in CKD patients, with strong diagnostic performance across different disease stages and in hemodialysis. Larger studies are warranted to further validate its utility.

Toshniwal et al. (2025) conducted a cross-sectional in Chronic kidney disease (CKD) (n=200). Hemodialysis vs. Pre-dialysis was evaluated on Prevalence of cardiac autonomic neuropathy (CAN) (p=0.01). Bellavere's Score demonstrated high diagnostic accuracy for cardiac autonomic neuropathy across chronic kidney disease stages, and hemodialysis significantly reduced CAN prevalence from 79.8% to 64%.
