Demonstrates improved occlusal function in mandibular retrognathia, suggesting a targeted surgical approach.
This study introduces a novel surgical strategy that uses a dento-osseous segmental osteotomy guided by digital planning to eliminate severe localized occlusal interference in mandibular retrognathia with asymmetry. Following a conventional BSSRO, this approach targets localized occlusal issues through precise osteotomy and repositioning of the dento-osseous segment bearing the interfering tooth, thereby simultaneously preserving the interfering tooth, establishing the intended functional occlusion, and correcting maxillofacial deformity. Postoperative outcomes showed satisfactory recovery of occlusal function with long-term stability, confirming the safety and efficacy of this approach. These findings propose a novel “problem‑focused” surgical strategy that prioritizes targeted, localized intervention to manage complex constraints and underscore the indispensable role of digital technology in its precise planning and execution.
