Surgical LAA exclusion and apixaban fail to prevent pouch thrombus, necessitating precardioversion TEE.
Why the study?
The optimal timing for stopping anticoagulation and the need for precardioversion echocardiography after epicardial LAA exclusion remain uncertain.
Does surgical exclusion of the left atrial appendage and anticoagulation obviate the need for precardioversion transesophageal echocardiography?

Does surgical exclusion of the left atrial appendage and anticoagulation obviate the need for precardioversion transesophageal echocardiography?
Surgical exclusion of the left atrial appendage and systemic anticoagulation do not eliminate the risk of thrombus formation, highlighting the continued need for precardioversion TEE.

Case suggests LAA exclusion plus anticoagulation may not eliminate thrombus risk; leaves open whether precardioversion TEE remains necessary.
Background: . Cases summary: (35 mm). He had recurrent atrial flutter/Afib postoperatively. He received apixaban in addition to rate control medications, and he was readmitted for precardioversion TEE which also demonstrated a LAA pouch thrombus. Discussion: is a stapler exclusion device via epicardial approach which has shown excellent exclusion rates in contemporary data. One of the pitfalls of this technique is the possibility of leaving a LAA remnant stump or pouch that is highly thrombogenic. The optimal timing for stopping anticoagulation and the need for precardioversion echocardiography remain uncertain.

Presti et al. (2022) conducted a case report in Atrial flutter/Atrial fibrillation (n=1). Left atrial appendage clip and apixaban was evaluated on Left atrial appendage pouch thrombus. Surgical exclusion of the left atrial appendage and apixaban anticoagulation did not prevent LAA pouch thrombus formation, necessitating precardioversion transesophageal echocardiography.
