The r-LMR equation with rescaled cystatin C improves P30 accuracy by ~7 points over CAPA.
Why the study?

Does the Lund-Malmö GFR equation adapted for rescaled cystatin C accurately estimate GFR in children and adults compared to creatinine-based equations?
The Lund-Malmö GFR equation can be successfully adapted for use with rescaled cystatin C, potentially reducing the need for population- and biomarker-specific GFR estimating equations.
May improve adult GFR estimation accuracy; extends r-LMR to rescaled cystatin C but leaves open need for prospective validation.
-accuracy and generally performed better than the corresponding single-marker equations. Our results show that the Lund-Malmö GFR equation can be adapted for use with rescaled cystatin C with performance that is similar to the best-performing equations based on rescaled creatinine. The generality of the applied biomarker rescaling principle implies that the future demand for population- and biomarker-specific GFR estimating equations can be expected to decrease substantially.

Björk et al. (2024) conducted a cohort in Kidney function / GFR estimation (n=10,020). r-LMR equation with rescaled cystatin C (r-LMRCys) vs. CAPA cystatin C equation was evaluated on P30 accuracy (percent estimates within ±30% of measured GFR) in adults (Difference 6.6 pp, 95% CI 5.8-7.4, p=<0.001). The r-LMR equation extended to rescaled cystatin C achieved a P30 accuracy of 86.2% in adults, which was 6.6 percentage points higher than the CAPA equation.
