Simulated combined IK1 and ICaL blockade prolongs APD and QT interval in SQT3 computational models.
Why the study?
While the KCNJ2 D172N mutation causing SQT3 has been widely studied, the potential effects of the KCNJ2 E299V mutation and ion channel blocking under this condition remain little known.
Does combined blocking of IK1 and ICaL prolong APD and QT interval in a computational model of KCNJ2 E299V mutation Short QT Syndrome?
Does combined blocking of IK1 and ICaL prolong APD and QT interval in a computational model of KCNJ2 E299V mutation Short QT Syndrome?
Computational modeling demonstrates that combined blocking of IK1 and ICaL prolongs action potential duration and QT interval, suggesting a potential therapeutic strategy for Short QT Syndrome variant 3.
Short QT syndrome (SQTS) is a cardiac disorder characterized by arrhythmia and even sudden cardiac death (SCD). SQTS variant 3 (SQT3) has been linked to the KCNJ2 gene mutations, which directly increasing the inward rectifier K⁺ current <tex>(I_K1 )</tex>. There have been many studies on the effects of the mutation KCNJ2 D172N that cause the SQT3, but the potential effect of the mutation KCNJ2 E299V is little known. Therefore, we aim to predict and compare the potential effects of ion channels blocking under the E299V mutation. In this study, a biophysically detailed computer model of the heart which was developed by was coupled with the KCNJ2 E299V mutant IK1 patch clamp data. Effects of a combined action of blocking of <tex>IK1</tex> and ICaL was also simulated under the E299V mutant condition. Our simulation data showed that a combined action of blocking of <tex>IK1</tex> and <tex>ICaL</tex> prolonged the cardiac cell action potential duration (APD) and QT interval under SQT3 E299V condition, and demonstrated that blocking of <tex>IK1</tex> and <tex>ICaL</tex> produced a therapeutic effect under SQT3 E299V. This study provides new evidence that blocking of <tex>IK1</tex> and <tex>ICaL</tex> may be a potential treatment for SQTS patients.

Luo et al. (2021) studied Short QT syndrome (SQTS) variant 3 (SQT3) with KCNJ2 E299V mutation. Combined blocking of IK1 and ICaL (simulated) was evaluated on Action potential duration (APD) and QT interval. Simulated combined blocking of IK1 and ICaL prolonged the cardiac cell action potential duration and QT interval in a computational model of Short QT syndrome variant 3 with KCNJ2 E299V mutation.
