Summary Inguinal nodal disease is the most important prognostic indicator in penile cancer. The presence of microscopic nodal disease in clinically node negative penile cancer is most accurately assessed through surgical staging. Surgical staging with inguinal dynamic sentinel lymph node sampling (DSNB) reduces morbidity compared with other approaches to inguinal lymphadenectomy. Surgical staging with DSNB improves penile cancer survival and when performed in high-volume centres, DSNB has high sensitivity and low false negative rates.
