Many patients with heart failure should receive an aldosterone receptor antagonist, ie, either spironolactone (Aldactone) or the newer agent eplerenone (Inspra)--in addition to an angiotensin-converting enzyme (ACE) inhibitor or an angiotensin II receptor blocker (ARB) or both, and a beta-blocker.
