Treadmill CPET elicits greater physiological responses than 6MWT and TUG.
Why the study?
To compare physiological responses during a treadmill cardiopulmonary exercise test, 6-minute walk test, and timed up and go test in patients with unexplained breathlessness.

How do physiological responses compare between treadmill CPX, 6MWT, and TUGT in individuals with unexplained breathlessness?
Treadmill CPX elicits the greatest physiological response compared to 6MWT and TUGT, though TUGT still produces a significant physiological response despite its short duration.

Supports treadmill CPX when maximal responses are needed in unexplained breathlessness; hypothesis-generating for test selection in breathless patients.
Purpose: To compare physiological responses during a treadmill cardiopulmonary exercise test (CPX), 6‐minute walk test (6MWT), and timed up and go test (TUGT) in individuals referred for unexplained breathlessness and symptom limited treadmill exercise testing. Methods: Heart rate (HR), oxygen consumption (V̇O 2 ), carbon dioxide production (V̇CO 2 ), respiratory exchange ratio (RER), minute ventilation (V̇ E ), systolic blood pressure (SBP), and rating of perceived exertion (RPE) were recorded throughout each test. Results: Each test demonstrated a significant increase ( p < 0.01) in the cardiopulmonary (V̇O 2 , V̇CO 2 and V̇ E , RPE, SBP, and HR) and perceptual (RPE) responses from rest to end exercise. The increase in cardiopulmonary and perceptual responses was greatest for the CPX with significantly smaller responses demonstrated during the 6MWT ( p < 0.01) and even smaller responses for the TUGT ( p < 0.01 vs CPX and 6MWT). Conclusion: Not surprisingly, the treadmill CPX results is the greatest physiological response in our group. Despite being of short duration, the TUGT results in an increased physiological response.

Alves et al. (2024) conducted an observational in Unexplained breathlessness. Treadmill cardiopulmonary exercise test (CPX) vs. 6-minute walk test (6MWT) and timed up and go test (TUGT) was evaluated on Physiological responses (HR, VO2, VCO2, RER, VE, SBP, RPE) from rest to end exercise (p=<0.01). Treadmill cardiopulmonary exercise testing elicited the greatest physiological responses compared to the 6-minute walk test and timed up and go test (p<0.01).
