Newly developed 32-item TOCAS-BF tool shows high reliability for evaluating older adult care transitions.
Why the study?
Existing tools often miss key facilitators and barriers to hospital-to-home transitions for older adults.
The TOCAS-BF is a reliable and valid 32-item tool for assessing barriers and facilitators during hospital-to-home transitions for older adults.
May inform transition planning for older adults; leaves open prospective validation before clinical adoption.
• Nurses can use TOCAS-BF to assess older adults' needs during hospital-to-home transitions. • Identifying barriers and facilitators helps nurses improve care, satisfaction, and outcomes. • Identifying barriers and facilitators aids in developing better policies for older adults. • Identifying facilitators and barriers improves outcomes and reduces readmission rates. Healthcare systems strive to improve hospital-to-home transitions for older adults, but existing tools often miss key facilitators and barriers. Therefore, this study aimed to develop a tool for assessing the facilitators of and barriers to effective transitional care for older adults and to evaluate its psychometric properties. A methodological study was conducted from September 2023 to February 2024 with 229 older adult volunteers to develop and test the tool. The Transition of Care for Older Adults Scale is a 32-item scale with a 4-factor structure accounting for 52 % of the total variance explained. The indicators of confirmatory factor analysis were satisfactory. Cronbach's alpha coefficient for reliability was 0.925, indicating an optimal score. The Transition of Care for Older Adults Scale supports nurses in improving discharge planning, follow-up, and family support, while aiding educators in teaching strategies and researchers in studying care models and policies.

Dolu et al. (2025) studied Hospital-to-home transitions (n=229). Transition of Care for Older Adults Scale (TOCAS-BF) was evaluated on Psychometric properties (factor structure and reliability) (Cronbach's alpha 0.925). The Transition of Care for Older Adults Scale (TOCAS-BF) was developed as a 32-item tool with a 4-factor structure explaining 52% of variance and high reliability (Cronbach's alpha 0.925).
