Objective To evaluate the clinical efficacy and safety characteristics of a novel microfocused ultrasound system (MFUS) in the treatment of facial aging in Asian patients. Methods This single‐center, retrospective cohort study enrolled 107 Asian patients who were treated with MFUS (November 2021–October 2024). Standardized assessment tools, the Wrinkle Severity Rating Scale (WSRS), the Alexiades Comprehensive Grading Scale (ACGS), the Global Aesthetic Improvement Scale (GAIS), and the modified Manchester Scar Scale (mMSS), were used to quantitatively evaluate improvements in multiple facial regions, including marionette lines, tear troughs, midcheek grooves, nasolabial folds, and the jawline. Treatment‐related adverse reactions were systematically recorded. Univariate and multivariate regression analyses were used to explore the factors influencing the efficacy. Results Mean age was 39.5 ± 7.1 years. At 3 months, 85.3% achieved moderate or better improvement. The scores of various facial regions significantly improved: WSRS scores of 1–2 increased from 60.8% to 94.1% (marionette lines) and from 73.8% to 91.2% (tear troughs). ACGS scores improved from 32.6% to 68.1% (nasolabial folds) and from 14.9% to 83.6% (jawline). At the 6‐month follow‐up, the efficacy was well maintained, with the moderate or better improvement rate reaching 74.4%. Two treatments yielded higher significant improvement (62.5% vs. 29.2%) and greater satisfaction (45.0% vs. 9.2%) compared with a single treatment. Transient erythema/edema occurred in 89.7% and mild pain (Visual Analog Scale VAS score, 1–3) in 71.0%. Among patients with melasma, improvement declined from 63.8% at 3 months to 36.0% at 6 months. There was no significant correlation between treatment parameters and efficacy, while age was negatively correlated with efficacy. Conclusion This study confirms that MUFS can safely and effectively improve facial aging in Asian populations.
